Guidelines for Hospitals
- We will accept samples from hospitals, healthcare institutions, quarantine centres and do home collection of samples. No walk- ins, No CC collection, No DC/Corp collection
- Sample acceptance is based on "First come first serve basis". Hence, hospitals intending to send samples need to book the test in advance.
- Our Covid test booking centers are Phoolbagan, Barasat, Dunlop, Kestopur, Salt Lake, Kasba, Howrah & Elgin Road
- Booking at above centers will be done on the basis declaration from hospital as per attached format along with the payment.
- Hospital to send all the mandatory documents
a. Doctor’s prescription
b. Duly filled in Form 44 with stamp,
c. Approved patient ID card (aadhar, driving licence, passport, ration card, voter id etc) through email at covid19@surakshanet.com prior to sample dispatch.
- Hospitals must ensure proper sample storage till it reaches Suraksha. Test result is highly dependent on sample integrity.
- Responsibility of sample collection, transport to Suraksha receiving desk in Newtown is the sole responsibility of the concerned Client. Only authorized representatives of the concerned hospital with proper ID card and/or authorization letter (will be checked and verified at receiving desk) can carry and submit the sample.
- The sample transport box should be clearly marked as COVID 19 sample for easy and prompt recognition by Suraksha representative.
- Samples will be accepted along with a completed set of documents which are shared over mail.
- If the department feels that the SRF and sample (quality/quantity) is improper, test will not be run and hospital will be informed for sending fresh SRF and or sample as applicable. Suraksha's decision will be final in this regard.

Sample collection:
- Sample for COVID-19 testing should be collected only from those patients, who fulfil suspected COVID-19 testing criteria as per the ICMR guidelines.
- Hospitals should ensure that, sample is collected as per standard protocol only (MOHFW guideline) and that the sample is dispatched to Suraksha after proper packaging maintaining cold chain.
- Viral transport media (VTM) needs to be transported using the triple packaging protocol, with sufficient ice pack inside.
- VTM must only be the type, approved by the Government will be provided by Suraksha at the time of billing.
Reports:
Suraksha is committed to release reports at the earliest. However, depending upon the sample load and sample receiving time in department, TAT may vary from 24 to 48 hours. Hospital authorities are requested to co-operate with us in this regard.
